Eligibility  
 
 

If you or a loved one live in an area near Washington, Pennsylvania, and require ongoing medical care or support services, SeniorLIFE Washington may be able to help you.  Our member-based program is designed to provide and/or coordinate medical care and assistance needed to keep you or a loved one at home for as long as possible.  SeniorLIFE Washington is part of a State program and is an alternative for nursing home placement.  It has been implemented in many communities around the country and is now available in Washington.

SeniorLIFE Washington may be the right option if you or your loved one:

  1. Is 60 years of age 
  2. Resides in the area served by SeniorLIFE Washington
  3. Is determined to be Nursing Facility eligible
  4. Is determined by SeniorLIFE Washington to be able to be safely served in the community
  5. Is determined eligible for Medical Assistance or able to pay privately
